a. Transkrip Nilai dengan IPK S.Ked minimal 2,22 (Angkatan dibawah Tahun 2012
dan Konversi) dan
minimal 2,76 (Angkatan mulai Tahun 2012)
b. Surat Keterangan Bebas Akademik dan Keuangan untuk Sidang Hasil
Skripsi
c. Surat Keterangan Bebas Akademik dan Keuangan untuk Yudisium S.Ked
d. Surat Keterangan Bebas Laboratorium
e. Surat Keterangan Bebas Perpustakaan
f. Sertifikat lulus OSCE
g. Sertifikat Lulus PPA
Dokumen ini diserahkan melalui loket 8 P3T (Mas
Edi),sesuai paling lambat sampai jadwal penutupan. registrasi Calon Peserta
Yudisium.
